Symptoms
While trying the change the password for user, the password is prompted for 4 times(instead of two times).
[root@OL7 ~]# useradd <username1>
[root@OL7 ~]# passwd <username1>
Changing password for user <username1>.
New password:
BAD PASSWORD: The password contains the user name in some form
Retype new password:
Enter new UNIX password:
Retype new UNIX password:
passwd: all authentication tokens updated successfully.
Changes
Changes were made to the /etc/pam.d/system-auth file.
