Upgrade Oracle VM Server Using an ISO Reports Warnings About Inconsistent LVM volumes (Doc ID 2494216.1)

Symptoms

Upgrading an Oracle VM Server from version 3.4.4 to 3.4.5 using an ISO file (or burned to physical media)  pops up a warning regarding inconsistent LVM volumes, similar to:

Error processing LVM.

There is inconsistent LVM data on Volume Group vgdb. You can re-initilalize all related PVs (/dev/mapper/multipathd, /dev/mapper/multipathf)

which will erase the LVM metadata, or ignore which will preserve the contents. This action may also be applied to other PVs with inconsistent metadata.

and the installer presents the options : Ignore , Ignore all, Re-initialize , Re-initialize all

Changes

 Trying to upgrade the server to Oracle VM version 3.4.5
