Oracle VM: OVM3 mkfs(8) On A Free Partition Fails With Error "/dev/sdX is apparently in use by the system; will not make a filesystem here!" (Doc ID 2520258.1)

Symptoms

 Attempting to create a dom0 filesystem onto a free partition using mkfs(8) fails with the error message:

# mkfs -t ext4 /dev/sda5
mke2fs 1.43-WIP (20-Jun-2013)
/dev/sda5 is apparently in use by the system; will not make a filesystem here!

 This is often seen when a user tries to create a kdump(8) file area with the partition mounted to /var/crash.

Changes

Trying  to format the first unused partition on a dom0 server.
