Grub fails to update after upgrading or installing a new kernel on Oracle Linux 6 server (Doc ID 2520727.1)

Symptoms

A server running Oracle Linux 6 has had new kernels installed or upgraded on it via yum, however the server continues to boot into the older, previously installed kernel,

and the new kernel does not appear in the grub boot list when booting the server.
