Qlogic qed-based NICs stopped working after upgrading to RHCK 3.10.0-957.el7 (Doc ID 2523553.1)

Symptoms

"Qlogic qed-based NICs not visible after upgrading server to Oracle Linux 7 update 6. 

The system logs shows the below messages:

Changes

 The server was patched from an older Oracle Linux OS to Oracle Linux 7.6. The kernel was upgraded from N older RHCK kernel to kernel-3.10.0-957.1.3.el7.x86_64.
