Oracle Linux : ISCSI Packages Upgrade Caused LUN Outage (Doc ID 2564486.1)

Symptoms

All iscsi volumes were disconnected after updating iscsi related rpm packages,  and also iscsi logins failed to work as expected immediately after the update.

The following is seen in the logs:
