Symptoms

A Cisco UCSC-C240-M4SX server running Oracle Linux repeatedly experiences hardware errors, with the following messages regularly logged to /var/log/messages:

[ 6967.996544] EXT4-fs error (device sds2): ext4_find_entry:1312: inode #11275073: comm python2.7: reading directory lblock 0
[ 6967.997407] EXT4-fs (sds2): previous I/O error to superblock detected
[ 6967.998302] sd 0:2:0:0: [sds] tag#0 FAILED Result: hostbyte=DID_NO_CONNECT driverbyte=DRIVER_OK
[ 6967.998306] sd 0:2:0:0: [sds] tag#0 CDB: Write(10) 2a 00 00 10 08 00 00 00 08 00
[ 6967.998309] blk_update_request: I/O error, dev sds, sector 1050624


The server's LSI Logic / Symbios Logic MegaRAID Controller and associated direct attached SAS storage, however, do not show any errors.
