Oracle VM: Fail to Login VM from OVM Server via 'xm console' with error: Login incorrect (Doc ID 2571013.1)

Symptoms

In Exadata 19.2.2.x, customer can login VM with user 'root' via SSH, but fails to login VM from OVM Server via 'xm console' by hitting error 'Login incorrect'.

File /var/log/secure shows following failures

Jul 9 16:27:43 VMguest login: PAM unable to dlopen(/lib/security/$ISA/pam_tally2.so): /lib/security/../../lib64/security/pam_tally2.so: cannot open shared object file: No such file or directory <<----
Jul 9 16:27:43 VMguest login: PAM adding faulty module: /lib/security/$ISA/pam_tally2.so
Jul 9 16:27:43 VMguest login[10671]: pam_vas: Authentication <ignored> for <Non-VAS> user: <root> account: <N/A> service: <login> reason: <account is not a VAS or Mapped account> Access Control Identifier(NT Name):<N/A>
Jul 9 16:27:46 VMguest login[10671]: FAILED LOGIN SESSION FROM ttyS0 FOR root, Module is unknown <<-----
 

Changes

Customer made the changes in file /etc/pam.d/system-auth
