Oracle Linux : Server Fails To Boot - 'kernel: usnic_verbs:usnic_ib_init:616: Unable to initalize umem with err -1' (Doc ID 2614857.1)

Symptoms

An Oracle Linux Cisco UCS blade server without an Infiniband(IB) Host Channel Adapter (HCA) installed reports a fatal error during boot. System logs denote the following error:

/var/log/messages

localhost kernel: [ 10.630678] IPv6: Loaded, but administratively disabled, reboot required to enable
localhost kernel: [ 10.637141] usnic_verbs: Cisco VIC (USNIC) Verbs Driver v1.0.3 (December 19, 2013)
localhost kernel: [ 10.637144] usnic_verbs:usnic_uiom_init:587: IOMMU required but not present or enabled. USNIC QPs will not function w/o enabling IOMMU
localhost kernel: [ 10.637461] usnic_verbs:usnic_ib_init:616: Unable to initalize umem with err -


/var/log/boot.log

Loading the OpenIB kernel modules: FATAL: Error inserting usbnic_verbs (/lib/modules/4.1.12-124.16.4/kernel/drivers/inifiband/hw/usnic/usnic_verbs.ko): Operation not permitted
