Symptoms

A server running Oracle Linux 6 crashed with the following errors:

Jan 17 18:05:54 <hostname> kernel: ADVMK-0020: A read error was reported to the ASM instance for volume VOLUME in diskgroup DISKGROUP
Jan 17 18:05:54 <hostname> kernel: ACFSK-0058: I/O failure 0xc0000185 with device /dev/asm/DEVICE during a metadata synch read.
Jan 17 18:05:54 <hostname> kernel: ACFSK-0059: Fenum Unknown. Starting offset: 72192 . Length of data transfer: 512 bytes.
