Symptoms

ocfs2 module cannot be loaded and failing to bring the cluster up with the below error messages.

# /sbin/o2cb.init status

Driver for "configfs": Loaded
Filesystem "configfs": Mounted
Driver for "ocfs2_dlmfs": Not loaded
Checking O2CB cluster "ocfscluster": Offline
Debug file system at /sys/kernel/debug: mounted
# /sbin/o2cb.init start

Loading filesystem "ocfs2_dlmfs": Unable to load filesystem "ocfs2_dlmfs"
