Symptoms

An Oracle Linux 6 iSCSI initiator and target are configured for iSCSI mutual / bi-directional CHAP authentication.
This configuration requires the iSCSI target to authenticate the initiator and for the iSCSI initiator to authenticate the target.
Having successfully discovered the iSCSI target, attempting to login to the iSCSI target from the initiator fails with the following error:
