Oracle Linux: Node crashes or hung with error: "xfs/filesystem errors: I/O Error Detected. Shutting down filesystem" (Doc ID 2727573.1)

Linux OS - Version Oracle Linux 7.2 with Unbreakable Enterprise Kernel [4.1.12] and later
Linux x86-64

Symptoms

Server hangs or crashes with either kernel BUG at fs/xfs/xfs_icache.c:321! or xfs error "I/O Error Detected. Shutting down filesystem". Issue usually seen when there is high IO or while taking the backups. 
The error "I/O Error Detected. Shutting down filesystem" is getting reported around the issue time. Running the filesystem check tool xfs_repair(8) manually does not detect a problem.

 

Changes

 Issue observed after upgrading to 4.1.12-124.40.6/4.1.12-124.42.3.el7uek.x86_64
