Oracle Linux: pam_tally2 Showed Failed Attempt After Entering Username (Without Entering Password Yet) (Doc ID 2767671.1)

Symptoms

When logging in to the system via ILOM console, pam_tally2 shows that there's a failed attempt after typing in username (no password yet).

Since pam.d was configured to lock user temporarily for XXX secs every failed attempt.  As a result, unable to login via console.
