System Crash with "qla2xxx [0000:xx:xx.x]-802b:0: BUS RESET SUCEEDED nexus=x:x:xx" (Doc ID 2780299.1)

Symptoms

A system crash or an unexpected reboot occurs. In the dmesg log with last console terminal messages before the event it is found some error messages related to IO like the:

INFO: task <PROCESS> blocked for more than 120 seconds.
echo 0 > /proc/sys/kernel/hung_task_timeout_secs" disables this message.

and in the last message shows a:

qla2xxx [0000:xx:yy.z]-802b:0: BUS RESET SUCCEEDED nexus=x:x:xx

In the lspci command output it is confirmed that the device ID from the last message, xx:yy.z is a QLogic Card.

Eg:

The dmesg shows:

qla2xxx [0000:12:00.0]-802b:0: BUS RESET SUCEEDED nexus=0:0:27.

In lspci shows that device ID 12.00.0 is:

12:00.0 Fibre Channel [0c04]: QLogic Corp. ISP2722-based 16/32Gb Fibre Channel to PCIe Adapter [1077:2261] (rev 01)
