OLVM: VDSM And VMs Die With IO Errors After Software Update On IBM SVC. (Doc ID 2782840.1)

Symptoms

OLVM cluster is connected to a centralized IBM Storage Cluster. VDSM nodes boot from SAN. After software update on one of the two SVCs (Storage Volume Controller), VDSM nodes lose paths to storage and all VMs die.

Physical console of VDSM nodes is filled by the following error:

blk_update_request: I/O error, dev dm-xx, sector xxxxxx op 0x0: (READ) flags 0x0 phys_seg 256 prio class 0
