Symptoms
FC link over QLogic FC HBA sometimes sees port flaps with FC port going from LOST to DEAD state and does not recover back to ONLINE. This happens intermittently causing IO disruption, if the IO is a heartbeat for the cluster node, the disruption causes the node to be evicted.
