OLVM: Oracle Linux 5 VM Hangs with Error "hda: Drive Not Ready For Command" (Doc ID 2796165.1)

Linux x86-64

Symptoms

Environment: Oracle Linux Virtualization Manager version 4.3.6 or above.
Some Virtual Machines running Oracle Linux 5 may lose access to the root disk with consequent panic and crash.
