Oracle Linux 7: UEK5 System Crashes After Fiber Channel Storage Cable Removal Testing (Doc ID 2800309.1)

Symptoms

An Oracle Linux 7 server installed with QLogic Fiber Channel (FC) Host Bus Adapter (HBA) cards is connected to a FC Storage Array via multiple cables.
One FC cable connecting the server and storage is removed to test storage High Availability (HA).
Once removed, the system crashes with the following call stack trace:
