OLVM: I/O Error When Writing on a Direct Attached Disk with Pass-Through Enabled (Doc ID 2846670.1)

Symptoms

When trying to create a new file system, or writing in a existing file system, the following messages can be observed in /var/log/messages:

Dec 20 08:08:15 <hostname> kernel: Buffer I/O error on dev sda, logical block 3, lost async page write
Dec 20 08:08:15 <hostname> kernel: Buffer I/O error on dev sda, logical block 4, lost async page write
Dec 20 08:08:15 <hostname> kernel: Buffer I/O error on dev sda, logical block 5, lost async page write
