Symptoms

Oracle Linux with Linux 2.6.39-400.17.1.el6uek.x86_64 hung with soft lockup at mptspi_qcmd
There are hard disk device reset and error handling events shown

 "TEST_UNIT_READY" failed message can be seen from dmesg output.
