New ASMLib / oracleasm Disk Gets "header_status=Unknown" - Cannot be Added to ASM Diskgroup (Doc ID 391136.1)

Symptoms

After creating/labelling new ASM disks via ASMLib, the ASM instance V$ASM_DISK view reports the header_status of the disk as "UNKNOWN" instead of "PROVISIONED".
Consequently, the new ASM disks cannot be added to an ASM diskgroup.
Attempting to rescan the devices via ASMLib fails with:
