Symptoms

In some kernel versions, the default I/O scheduler CFQ (Completely Fair Queuing) has a known bug which can cause erratic system behavior, including a system reboot.  Often the first indication of this problem is that OCFS2 will fence the node because of disk heartbeat timeouts after a long I/O waits.
