Starting Oracle VM guest gives Error: Device 768 (vbd) could not be connected (Doc ID 742003.1)

Symptoms

When you attempt to start a guest with either VM Manager, or from the command line, you encounter the following error:

# xm create vm.cfg
Using config file "./vm.cfg".
Error: Device 768 (vbd) could not be connected. /etc/xen/scripts/block failed; error detected.
Note: this only occurs if you are using NFS storage for your /OVS repository, so if you recently changed from OCFS2 to NFS storage, the error may appear after this change.

Changes

The machine was recently upgraded from 2.1.1 to 2.1.2 using:

   # up2date --update
