No More Than 12 Oracle VM Servers Can Be Added To Same Server Pool (Doc ID 878475.1)

On Oracle VM Manager you are going to build a server pool with 14 servers. All servers are configured with a shared root repository (/OVS) and some other shared repositories. All repositories are mounted as OCFS2 on SAN storage. A typical mount output on the server could be:
#mount | grep OVS
/dev/mapper/ovsroot on /OVS type ocfs2 (rw,_netdev,heartbeat=local)
/dev/mapper/uuid1 on /OVS/uuid1 type ocfs2 (rw,_netdev,heartbeat=local)
/dev/mapper/uuid2 on /OVS/uuid2 type ocfs2 (rw,_netdev,heartbeat=local)
/dev/mapper/uuid3 on /OVS/uuid3 type ocfs2 (rw,_netdev,heartbeat=local)
/dev/mapper/uuid4 on /OVS/uuid3 type ocfs2 (rw,_netdev,heartbeat=local)

Everything goes fine until adding the thirteen server. When click the 'add' button to add the thirteen server, the Manager web page returns normal. However, you cannot see the new added server in the server list.

You checked the /var/log/ovs-agent/ovs_operation.log on the pool master, but the server is successfully registered:
"2009-08-19 02:40:17" INFO=> register_server: your.server.hostname->utility,xen
