The image from the header and/or footer does not appear in the generated PDF output even if the image is present in the RTF template.

The issue can be reproduced at will with the following steps:
1. Create any BI Publisher, (XML Publisher), template using the Word XMLP plugin that includes a report header and footer.
2. Preview the report.
3. Observe that the image just above the footer is NOT shown in the generated PDF output.
