In Self Service Outreach on My Reminders Page, When User Selects 'Save As Default', Get Error No Registry Entry for Component in SCC_ATTACH_IMP (14015,2001)

Symptoms

After applying CS 9.0 Bundle #26, when setting up and using the Outreach self service menu items, if a user accesses the 'My Reminders' page and clicks the 'Save as Default' button, the following error is encountered:

No registry entry for component in SCC_ATTACH_IMP (14015,2001)

Contact technical support.  For each component that uses attachments a row must be stored in the table
SCC_ATTACH_IMP.  This row tells the attachment framework how to process its requests.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. Navigate to Self Service > Outreach > My Prospects > My Action Plans > My Reminders (Component: AV_MYREMINDER_CP; Page: AV_MYREMINDERS).
2. Update the Number of Upcoming Days to 30.
3. Click on the 'Save as Default' button.

Due to this issue, users cannot use the save as default functionality on the My Reminders page.
