E-AS PSAPPSRV Crashes at SmServerConnection::EnqueueWaiter with Custom Signon PeopleCode (Doc ID 1517827.1)

Symptoms

Users were unable to login to the Peoplesoft application and were getting the below error message at the login page :

bea.jolt.ServiceException: TPESVCERR - server error while handling request

The PSAPPSRV log file generated the following :
