CRM - Ad Hoc Notification Sends Multiple duplicate Emails (Doc ID 1588405.1)

Symptoms

ISSUE:

Ad Hoc Notification Sends Multiple duplicate Emails when the email has attachment being sent to a single recipient.  Outbound emails shows just 1 email but recipient gets 3 emails.  If sending without an attachment, then user receives only 1 email.

The app server log shows:

PSAPPSRV.9465 (105)  (FIREFOX 9.0.1; WIN7) ICPanel](0)     ExecuteGetAttachment (error): specified URL (/100025555331Sheaffer,_Emily.doc) does not start with a supported destination-indicating prefix.
PSAPPSRV.9465 (105)  (FIREFOX 9.0.1; WIN7) ICPanel](0)     Preceding error is unrecoverable and will result in termination of the current process.

STEPS

1. Go to any Component that ad hoc notification can be sent
2. Click on the Notification Tab to bring up the Send Notification Page.
3. Send an email with an attachment to a single recipient
