EAP WTHD: How to populate Box 11 and Box 12 for 1099's

Base on the description of 2013 1099 fix, Box 11 and Box 12 are added.
Does the US_REPORT (under Setup Financials/Supply Chain > Product Related > Procurement Options > Withholding > Report Definition) have to be updated to add Box 11 and Box 12? If is is required, what are the values that need to be put in them?
