EAP: Phone information for Vendor Contact (PS_VNDR_CNTCT_PHN) is saved with invalid Effective Date so Phone is blank (Doc ID 1616604.1)

Symptoms

The Phone Information for the Vendor Contact (PS_VNDR_CNTCT_PHN) is saved with an invalid Effective Date.  As a result, the Phone Information is not displayed correctly (it is blank) on the Vendor > Contact page.

STEPS
1.  Currently, Vendor ID = SCM0000011 has the following information:
 Address ID 1:  Effective Date = 07/26/2000; Phone = 11111111
 Address ID 2:  Effective Date: 01/01/2013; Phone = 22222222

 Vendor Contact 1:  Address ID 1: Effective Date: 07/2607/2000; Phone = 33333333
 Vendor Contact 2:  Address ID 2: Effective Date: 01/01/2013; Phone = 44444444

2)  Add another Contact
 Vendor Contact 3: Address ID 2: Effective Date: 10/03/2013
 Telephone (22222222) was filled out automatically when populating Address field = 2

3)Tables:

VENDOR_ADDR_PHN:
Address ID 1: Effective Date: 07/26/2000 Phone = 11111111
Address ID 2: Effective Date: 01/01/2013 Phone = 22222222

VENDOR_CNTCT:
Vendor Contact 1 Effective Date: 07/26/2000
Vendor Contact 2 Effective Date: 01/01/2013
Vendor Contact 3 Effective Date: 10/03/2013

VNDR_CNTCT_PHN:
Vendor Contact 1 Effective Date: 07/26/2000; Phone = 33333333
Vendor Contact 2 Effective Date: 01/01/2013; Phone = 44444444
Vendor Contact 3 Effective Date: 01/01/2013; Phone = 22222222, which is incorrect.  Effective Date for Vendor Contact 3 (PS_VNDR_CNTCT_PHN.EFFDT) should be 10/03/2013 instead of 01/01/2013.

4) Re-display Vendor ID = SCM0000011
Vendor Contact 1 Effective Date: 07/26/2000; Phone = 33333333
Vendor Contact 2 Effective Date: 01/01/2013; Phone = 44444444
Vendor Contact 3 Effective Date: 01/01/2013; Phone = BLANK, which is incorrect.  It should be 22222222.
