Emails Using Generic Template EP_APPR_HEADER_APPROVED Not Getting Generated (Doc ID 1988043.1)

Symptoms

Issue :  When a Document is completed (Final approval) the email to be generated using the EP_APPR_HEADER_APPROVED template is not sent to the Requester.  The setup on the approvals - Transaction Configuration page is all done correct, however, emails are not getting generated. The users, however are receiving the email created by the Generic template email EP_APPR_APPROVAL_REQ.  

Also please clarify when exactly the email from the EP_APPR_HEADER_APPROVED generic template is to be triggered - Once the final approval is completed in the Approval Framework or when the Complete button is pushed on the performance document ?
