AR9.2: Item Not Found (15750,4106) On Collections Workbench Add Conversations

When attempting to Add Conversations On Collections Workbench, the following error occurs.
ERROR ----------------------- Message Item not found (15750,4106) The Quality Server was unable to find the correct item. Exit this page and try again
STEPS ----------------------- The issue can be reproduced at will with the following steps: 1. Navigate to Collections Workbench page and search for customer. Click on ‘Add Conversations’ link; 2. Enter Subject, Description , Comments; 3. Click on References Tab. Enter as qualifier as I and click on prompt for Reference ID. Search for Item Id; 4. Select item ID (Line 1). The Business Unit, Customer ID are copied but not the item Line 5. Click on Apply end the error occurs.
