AR9.2: Item Not Found (15750,4106) On Collections Workbench Add Conversations

When attempting to Add Conversations On Collections Workbench,
the following error occurs.
Item not found (15750,4106)
The Quality Server was unable to find the correct item. Exit this page and try again
The issue can be reproduced at will with the following steps:
1. Navigate to Collections Workbench page and search for customer. Click on ‘Add Conversations’ link;
2. Enter Subject, Description , Comments;
3. Click on References Tab. Enter as qualifier as I and click on prompt for Reference ID. Search for Item Id;
4. Select item ID (Line 1). The Business Unit, Customer ID are copied but not the item Line
5. Click on Apply end the error occurs.
