ETL9.2 Reported Time Shows Awaiting Further Approval After Manager's Approval (Doc ID 2058521.1)

Reported Time shows Awaiting Further Approval after manager approval

We were originally on PUM Image 9 and installed PUM Image 12 on our environment as suggested by Oracle in-order to get this bug fixed but we continue to face this issue.
Attached is screen shots of timesheet and approvals. I have also attached results from SQL queries.

Reported time should be routed correctly & when manager approves time it should go to Approved status instead of awaiting for further approval.

The issue can be reproduced at will with the following steps:
1. Enter timesheet.
2. Login as Manager
3. Approve time
4. it shows time approved.
5. On Monitor approval only first row goes to the status Needs Approval rest of the rows goes into Awaiting for further approval when there are only 1 level approval is setup by using tlbyposnsupervisor

The issue has the following business impact:
Due to this issue, Managers are unable to approve the time, This is impacting their go-live as basic functionality is not working.
