After applying PUM Image 15, when attempting to add a Dependent/Beneficiary in Self Service, the following error occurs:


Steps to replicate the issue: 

1. Attempt to add a Dependent/Beneficiary in Self Service (Navigation: Self Service > Benefits > Dependent/Beneficiary Info).
