E-PIA: Login to PIA Keeps Looping, Error "redirected you too many times" (Doc ID 2155093.1)

When logging in to PIA, the same request gets looped over and over.  The problem is specific to an environment behind a load balancer or other network device.

Other symptoms include:
- When checking a Fiddler trace or network capture, the homepage URL (i.e. /h/?tab=DEFAULT) gets requested over and over
- With Chrome, you may get an error such as "<hostname> has redirected you too many times"
