On Manage Service Indicator Page, Operator ID for Assigned By Does Not Match Displayed Name

On Manage Service Indicator page, Operator ID (Assigned by) does not match name.

When Manage Service Indicators page is viewed, Peoplecode does not appear to distinguish between name of operators who have added the same service indicator. It is picking up the correct operator, but incorrect name (name of first operator to add this particular service indicator code).

The issue can be reproduced at will with the following steps:
1. Go to Campus Community > Service indicators (Student) > Manage Service Indicators.
2. Note that the operid does not match the name at times.

The issue has the following business impact:
Due to this issue, users cannot see correct user name at times.
