Display Name Not Visible After Applying Global Name Enhancements

Information in this document applies to any platform.
When attempting to complete a Name Change via Self Service, the Display Name is not visible/ blank.
This occurs in a PUM Image #19 instance where the 2016 ACA Fixes have not been applied.
Display Name should appear in Name Change component.
The issue can be reproduced at will with the following steps:
1. Navigate: Self Service > Personal Information > Name Change
2. Display Name Blank
