ACA Self Mailer PDF File Was Not Created

We've ran the Create ACA XML Files process to print the 1095C self mailer PDF file. The process appeared to run fine, however the self mailer versions were not created but rather only the EE versions of the 1095.
The issue can be reproduced at will with the following steps:
1. Run Create ACA XML Files process for 2016. Navigation: Benefits, ACA Annual Processing, Transmittal and Forms, Create ACA XML Files
