Error (180,236) when Submitting a Name Change through Self Service (Doc ID 2249798.1)

Symptoms

After applying the most recent ACA fixes from PUM 19-21, when submitting a Name Change the following error is received:

First operand of . is NULL, so cannot access member RECNAME. (180,236) EOAW_CORE.NotificationManager.OnExecute Name:LoadTemplateData PCPC:1552 Statement:38

The issue can be reproduced at will with the following steps:
1. Navigate to Main Menu > Self Service > Personal Information > Name Change
2. Edit Name and Submit changes
3. Note the above error message is received

This error was also reported for the Address Change transaction in <Document 2246943.1>
