FLUID ADDRESS CHANGE SELF-SERVICE TRANSACTION EMAIL SHOWS FIELDS NOT VALUES

Self-Service Address change Transaction triggers Email and it shows fields not the actual Values.
The issue can be reproduced at will with the following steps:
1. Login in to Application from ESS Home page click on personal details.
2. Click on address and Change address then click Save.
3. Employee get an email formatted correctly:
4. But, HR gets an email that have %2 %3 %4 .. in email where fields are not filled in.
