The 'Change Name' button does not appear on Classic mode of Personal Information Summary Page from Self-Service component.
The issue can be reproduced at will with the following steps:
1. Login to PIA (CLASSIC)
2. Navigate to Self-Service -> Personal Information -> Personal Information Summary
3. 'Change Name' button does not appear
