Classic Name Change Transaction Fails When Blank Name Format is Selected

When a self service user enters a name change in Classic, non-fluid mode, the name format drop down shows a blank option in addition to other active configured values such as English ('001'). The user can select and submit a name change with a blank name format. As this is not a valid name change type, it eventually causes an error after the transaction is processed.
On Workforce Administration > Self Service Transactions > Name Changes
Further Processing Required Errors were encountered while this transaction was trying to save to the database.
The issue can be reproduced at will with the following steps:
1. In Classic mode
2. Self Service > Personal Information > Name Change
3. Select blank option in drop-down for Name Format
4. Complete transaction
5. Errors occur because blank option is not valid
