Chat with Agent Link pops error when employee tries to initiate chat

RC_SELF_SERVICE.DISP_TMPL_FAM_CD not found in the scroll Level 0 (2,155)

The issue can be reproduced at will with the following steps:
1. HR Service Center
2. Known Issues
3. click 'Chat with Agent' link
4. fill in some texts
5. click 'start chat'
6. get error
