E-CA: Data Conversion Error During Application Upgrade - "Process Server Name has not been defined in environment configuration for <process_scheduler_server_name>"

ENVIRONMENT

PT 8.55.13
Upgrade from FSCM 9.1 to FSCM 9.2


When attempting to run "Performing Data Conversion Concurrently" step from CA during application upgrade (FSCM 9.1 to FSCM 9.2) the following error occurs.

Mon Oct 02 22:04:03 CDT 2017
Error: Unable to execute step.
  Process Server Name has not been defined in environment configuration for SERVER1


STEPS
======
The issue can be reproduced at will with the following steps:
1. Configure CA for application upgrade
2. Run "Performing Data Conversion Concurrently" step
