After Upgrade to Campus Solutions 9.2, When Mass Assignment Process is Run for Activity Guides, Get "Action Item Already Exists. Please Provide a Different Action Item ID." (Doc ID 2345839.1)

Symptoms

After upgrading to Campus Solutions 9.2, users receive an error when testing the mass assignment of an activity guide.  As an example, the template is a clone of the classic, not fluid, Program Registration template. Rather than mass assigning the agreements as expected, the process starts but then stops with the following error:

Action item already exists. Please provide a different Action Item ID.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Assign a registration agreement that is based on the delivered Program Registration template, with a few minor changes.
2. Run the SCC_TM_ASIGN process to mass assign the task to all term activated students.
