SCM and DocuSign integration - User Authentication & Parameters

Unable to trigger the SMS User Authentication option for DocuSign eSignature. The Subject also does not pass to DocuSign (see image).

1. User Authentication (PRIORITY ISSUE)
applied bug fix: Bug 25250079 : DOCUSIGN RECIPIENT AUTHENTICATION FOR ACCESS CODE ALSO PROMPTS
FOR ID CHECK. After this bug fix was applied, we were able to send Access Code authentication. However, Phone and SMS does not trigger Recipient ID Check.

STEPS

The issue can be reproduced at will with the following steps:
1.   Created a document for signature using docusign
2.   Send document for signature

4.   Set up Identity Authentication page for SMS and PHONE

5.  Both of these do not work

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot send Identity Authentication using SMS or PHONE
