Symptoms

ACTUAL BEHAVIOR
---------------
When a student has more than one of the same checklist item codes (in this example, the student has two GEN checklist items, each from a different school) showing on the To Do List in the delivered tasks tile, the Organization Description shows the same school for each of those items instead of each item being tied to their respective organizations.

EXPECTED BEHAVIOR
-----------------------
Organization descriptions should display correctly in fluid mode.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Assign Checklist to Student ID
2. Login as Student in Fluid Mode
3. Click on Task Tile
4. Click on Item Description

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot able to view correct organization information.
