Create Case Button below MCF Toolbar Not Working

ACTUAL BEHAVIOR
---------------

When My HRHD Cases tile is selected on the Fluid Homepage, the page gets loaded perfectly with UAD Toolbar but the button below the toolbar such as Create Case does not work.

EXPECTED BEHAVIOR
-----------------------
Button should be responsive and should open the New Case page.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Logon to CRM 9.2 PUM 14 as JMENENDEZ (Configured MCF Agent)
2. Click on My HRHD Cases tile on the homepage
3. The page gets loaded with MCF/UAD Toolbar.
4. Click on Create Case button nothing happens.
