On : 9.2 version, Workflow

When an ad-hoc reviewer is inserted into a transactions workflow (initiated through MSS) and the workflow is saved, adhoc reviewer does not get an email or flag notification in the system.
The workflow acts as if ad-hoc reviewer never got added into the process. Why is this happening?
